Job summary

Senior Care Solutions Group is seeking a compassionate Physical Therapist (PT) with lymphedema certification to join our Medicare-certified home health team in Florida. The role focuses on delivering patient-centered care in patients' homes, improving mobility, reducing swelling, and promoting independence.

Responsibilities include evaluations, developing care plans, Medicare-compliant documentation, patient and caregiver education, and collaboration with the care team.

Qualifications

• Active Physical Therapist license in the state of practice.
• DPT or equivalent degree from an accredited program.
• Certified Lymphedema Therapist (CLT) certification.
• 1+ year home health experience preferred.
• Strong knowledge of Medicare Part A home health regulations and documentation requirements.
• CPR certification current.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive physical therapy evaluations per physician orders and Medicare guidelines.
• Develop and implement individualized care plans focused on functional mobility and lymphedema management.
• Monitor and document patient progress in EMR per agency standards.
• Educate patients and caregivers on home exercise programs and self-management.
• Collaborate with nursing, OT, ST, MSW, and physicians.
• Contribute to OASIS assessments and care coordination.
• Ensure timely documentation and regulatory compliance.
