Responsibilities

  • Consult with patients to assess physical condition and symptoms.
  • Review medical history and referrals from doctors or surgeons.
  • Diagnose movement dysfunction by listening and observing movement.
  • Develop individualized treatment plans.
  • Coach patients and teach therapeutic massage to ease pain and stimulate healing.
  • Maintain patient records by tracking goals.
  • Utilize standardized assessments and evidence-based practice to support interventions.
  • Assess progress and adjust treatments in the clinical record per requirements.
  • Evaluate treatment outcomes and modify services based on outcomes.
  • Provide consultation to patients, families, and other providers related to physical disorders.
  • Communicate progress and plans with supervisor and health team members.
  • Provide comprehensive discharge summaries per regulatory requirements.
  • Document regulatory and clinical information in the electronic system and maintain records.
  • Participate in trainings, in-services, and conferences.
  • Able to supervise and support physical therapist assistants, rehab aides, and students.
  • Adhere to company policies, procedures, and code of conduct.
  • Maintain active State Licensure.

Principal Responsibilities

  • Maintain patient record by keeping track of goals
  • Utilize standardized assessments and evidence-based practice to support clinical interventions.
  • Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
  • Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
  • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
  • Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
  • Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
  • Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
  • Participate in facility and company required trainings, in-services and conferences.
  • Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
  • Adherence to Company required policy and procedures and code of conduct.
  • Active State Licensure
