Experience/Qualifications:

• Licensed Physical Therapist

Key Responsibilities:

• Screen and evaluate patients with physical functioning disorders and/or wound care, developing appropriate care plans following all regulatory and clinical standards.
• Provide rehabilitative, skilled, and medically necessary treatments to patients with physical disorders and/or wound care needs.
• Administer procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, supportive and assistive devices, wound care, etc.
• Consult and counsel patients, families, caregivers, and other providers regarding physical disorders.
• Prepare comprehensive discharge summaries in accordance with regulatory and clinical requirements.
