Physical Therapist
Boulder Community Health, Boulder, CO 80303

$52.15 - $74.23 an hour - Per Diem Nights, Various shifts and hours per week
**Range includes per diem and night differential. +$2.50/hr for weekend shifts.

Boulder Community Health is seeking a Physical Therapist who assumes responsibility and accountability for providing quality patient care through the provision of physical therapy services.

Qualifications

  • Current Colorado state license for Physical Therapy.
  • BLS certification required by the end of conditional period.
  • Experience evaluating neurologically involved patients and conditions.
  • Developing, executing, assessing, and modifying physical therapy interventions for neurologically involved patients.

Responsibilities

  • Evaluative skills: Select appropriate tests and measures to determine goals, problems, course of treatment, and therapeutic procedures/modalities.
  • Selects and establishes physical therapy diagnosis.
  • Planning skills: Formulate and maintain an individualized patient plan of care based on goals.
  • Implementation skills: Provide treatment using appropriate tools and demonstrate knowledge of patient management.
  • Assessment skills: Evaluate patient response and revise plan of care accordingly.
  • Serve patient populations from adolescents to older adults/geriatrics, adapting assessments and treatments as needed.
  • Promote effective communication and teamwork with colleagues, hospital staff, and other contacts.
  • Report and document evaluations, plans, treatment techniques, and patient progress.
  • Initiate and be responsible for problem solving and decision making related to patient care.
  • Act as an advocate for the patient.
