Job summary

Fairview Health Services is seeking a Physical Therapist to support acute care inpatient needs at Fairview St John’s Hospital in Maplewood, MN. Responsibilities include examination, planning, intervention, and collaboration with patients and families to deliver safe, evidence-based care across diverse populations.

The role offers 0.775 FTE with a schedule that includes Monday–Wednesday, 8:00am–5:30pm, plus 4-week weekend rotation and one holiday per year, along with full benefits such as

Qualifications

• Minnesota Physical Therapy License (or temporary license) in Minnesota.

Responsibilities

• Partner with patient and family during Evaluation/Re-evaluation.
• Gather pertinent data.
• Perform examination.
• Determine treatment diagnosis.
• Develop an individualized plan of care with patient and family.
• Analyze data to identify needs and risks.
• Determine prognosis for recovery.
• Establish goals with patient and/or caregiver.
• Coordinate care with interdisciplinary team.
• Establish discharge plan.
• Provide skilled intervention and implement plan of care.
• Adjust plan as status changes.
• Educate patient/caregiver as needed.

Job Overview

We are seeking a physical therapist to support acute care inpatient needs at Fairview St John’s Hospital in Maplewood!

The physical therapist will work with a variety of patient populations; including, but not limited to, ortho, neuro, general medical, general surgical, cardiac, oncology and medically complex. In collaboration with patients and families, the physical therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes.

• FTE: 0.775, (62 hrs per pay period).
• Schedule: Monday, Tuesday, Wednesday, 8:00am - 5:30pm and weekend (Saturday & Sunday) rotation every four weeks and one holiday per year.
• Candidate would be able to pick up additional weekday and weekend shifts, as available!

Responsibilities

• The physical therapist partners with the patient and family while performing the Evaluation/Re-evaluation

• Individualizes plan of care considering developmental level and cultural differences
• Implements and modifies plan of care based on reassessment and patient response
• Coordinates care and communicates effectively with interdisciplinary team for all aspects of patient care
• Establishes appropriate discharge plan
• Partners with patients and families to provide skilled intervention
• Implements plan of care
• Alters treatment/plan of care to reflect change in patient status and response to treatment
• Assesses patient and/or caregiver learning needs and provides appropriate education

Required Qualifications

• Physical Therapy License in the state of Minnesota or temporary license in the state of Minnesota
• Basic Life Support (American Heart Association or Red Cross)

Preferred Qualifications

• Doctorate of Physical Therapy
• 1+ year experience
• 1+ year acute care experience

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions.
