PER DIEM PHYSICAL THERAPIST (PT) HOME CARE POSITION AVAILABLE IN RIVERDALE & SURROUNDING ZIP CODES IN BRONX, NY

Outreach PT, OT, and SLP is a well-established, highly reputed private practice that has been serving older adults in New York for over 20 years. We specialize in providing in-home outpatient rehabilitation to individuals who may not be able to attend an outpatient clinic or prefer to receive therapy at home. Due to continued growth, we are looking for dedicated and passionate Physical Therapists (PTs) to join our team and work with the geriatric population.

Qualifications of Physical Therapist (PT):

1. Doctorate or Master’s degree in Physical Therapy
2. Valid NYS PT License
3. 1 year of experience working with older adults preferred
4. Home care experience is a plus

Responsibilities of Physical Therapist (PT):

1. Conduct comprehensive physical therapy evaluations

3. Provide therapeutic exercises, balance training, gait training, and functional mobility interventions
4. Train patients and caregivers in fall prevention strategies, safety awareness, and home exercise programs
5. Collaborate with OTs, SLPs, caregivers, and physicians for a holistic care approach

Pay: $60.00 - $72.00 per hour
