Job summary

Evolve Therapy Services is hiring compassionate Physical Therapists for an in-house Skilled Nursing Facility located in Owego, NY. Full Time, Part Time, or PRN positions are available to provide high-quality resident care.

Responsibilities include planning, organizing, directing physical therapy services, documenting and billing per CMS requirements, and communicating findings to residents, families and the interdisciplinary team.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program is required.
• Active Physical Therapy license in states of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Document and bill therapy services according to CMS expectations.
• Develop and implement treatment plans with short and long-term goals.
• Evaluate residents and communicate findings to the team and families.
• Complete compliant documentation in NetHealth EMR.
