Job summary

Evolve Therapy Services is seeking compassionate physical therapists for an in-house Skilled Nursing Facility in Owego, NY. The position offers Full Time, Part Time, or PRN options to fit your schedule and patient care needs.

You will assess residents, develop treatment plans, document in NetHealth EMR, and collaborate with an interdisciplinary team to ensure high-quality resident care.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents according to established standards and productivity requirements.
• Adhere to procedures for documentation and billing of physical therapy services.
• Demonstrate judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screenings to determine need for intervention/treatment.
• Develop treatment plans with short- and long-term goals and methods to achieve them.
• Interpret and communicate findings to residents, families, and interdisciplinary team.
