Job summary

Boost Home Healthcare is seeking a licensed Physical Therapist to provide in-home care in Fort Collins and surrounding areas. Flexible schedules (Full-Time/Part-Time/PRN) and day shifts are available.

Compensation is $68.00–$76.00 per visit, with benefits including health, dental, vision, 401(k), and professional development. You will evaluate patients, create care plans, conduct 30‑day re‑assessments, and coordinate with the care team to maximize mobility and independence.

Qualifications

• Graduate with a Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program.
• Active PT state license in good standing.
• Minimum 1 year home health experience preferred; OASIS documentation proficiency.

Responsibilities

• Deliver physician-prescribed in-home physical therapy.
• Perform initial evaluations and develop plans of care.
• Conduct 30-day reassessments and coordinate with care team.
• Complete Medicare OASIS documentation per guidelines.
• Supervise PTAs and support personnel as required.

Key Responsibilities (What You’ll Do)

• Deliver Quality In-Home Care: Provide physician-prescribed physical therapy to patients in their residence, helping them optimize mobility and functional independence.
• Patient Assessments & Care Plans: Perform initial evaluations, establish personalized plans of care, develop maintenance programs, and conduct 30-day reassessments.
• Interdisciplinary Collaboration: Participate in patient care planning alongside nurses, physicians, and home care staff to ensure holistic patient outcomes.
• OASIS Documentation: Complete accurate clinical documentation following Medicare guidelines and OASIS protocols.
• Team Supervision: Direct and supervise Physical Therapist Assistants (PTAs) and support personnel as required.

Qualifications & Requirements

• Education: Graduate with a Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited APTA/CAPTE program.
• Licensure: Active Physical Therapist (PT) state license in good standing.
• Experience: Minimum of 1 year of clinical physical therapy experience preferred (Home Health experience highly preferred).
• Documentation: Proficiency with Medicare OASIS documentation and EHR software.
• Core Skills: Strong clinical assessment skills, compassionate patient-first mindset, and excellent communication abilities.
