The position is full-time, offering a 40-hour workweek with a Monday–Friday schedule and rotating weekends, at a 117-bed skilled nursing facility providing long-term care and short-term rehabilitation.

Qualifications

• Doctor of Physical Therapy (DPT) or equivalent degree.
• Current Maryland Physical Therapist license.
• Strong clinical judgment and communication skills.
• Compassionate and patient-focused approach.
• Ability to thrive in a team-oriented environment.
• Strong organizational and documentation skills.

Responsibilities

• Conduct physical therapy evaluations and functional assessments.
• Develop individualized treatment plans based on patient goals and clinical needs.
• Provide skilled interventions designed to improve strength, mobility, balance, endurance, and independence.
• Treat patients across both short-term rehabilitation and long-term care.
• Track patient progress and update plans of care as needed.
• Complete accurate and timely clinical documentation.
• Educate patients and caregivers on therapeutic exercises, mobility, and safety.
• Collaborate with the interdisciplinary team to support positive patient outcomes.
• Promote patient safety and functional independence.
