Job summary

Beloit Health System is seeking a Physical Therapist to join the Lee Lane Physical Medicine team in Wisconsin. The PT evaluates patients, sets goals, and creates individual care plans within the department's scope of services.

Responsibilities include implementing therapy plans, coordinating with the care team, documenting progress, and educating patients and families. A WI license and CPR certification are required; a sign-on bonus is offered.

Qualifications

• Bachelor, Master, or Doctorate of Physical Therapy from an accredited school.
• Current Wisconsin Physical Therapist license at hire or prior to services.
• CPR/AED certification within 3 months of hire.

Responsibilities

• Evaluate patients and develop goals and a plan of care within scope of services.
• Implement and coordinate PT treatment with the overall plan and other professionals.
• Review plan of care and adjust program per department policy.
• Educate patients and families on functional outcomes and abilities.
• Document evaluation, treatment, progress notes, and discharge summaries.
• Provide supervision of PT Assistants as per rules and regulations.
• Communicate effectively with team, patients, families, and insurers.
• Prepare, maintain, and clean work area to ensure safety and hygiene.
• Discharge planning and home program provision when appropriate.
