The role involves thorough documentation, collaboration with facility staff, and supervision of Physical Therapy Assistants. A Wisconsin PT license is required and SNF experience is preferred.

Qualifications

• Licensed Physical Therapist with Wisconsin eligibility.
• Ability to evaluate and treat SNF patients per care plan.
• Strong documentation and communication skills.

Responsibilities

• Provide exceptional patient care in SNF settings.
• Evaluate new patients and develop plans of care with measurable goals.
• Treat patients and educate them on injury progress and outcomes.
• Document evaluations, re-evaluations, discharges and daily progress notes.
• Collaborate with facility staff to ensure high-quality care.
• Supervise Physical Therapist Assistants and adjust plans as needed.
• Re-evaluate progress and revise goals accordingly.
