Job Details

The Staff Level II Physical Therapist is responsible for the evaluation, treatment, and discharge planning of all patients referred for their therapy services. The PT is an essential healthcare provider dedicated to helping patients reach their highest level of function. The therapist will uphold the mission, vision, and values of Jefferson Health and collaborate with other healthcare professionals to coordinate treatment plans and care.

Job Description

Essential Functions

  1. Provide therapy evaluations, treatment planning, interventions, and discharge planning for a full patient caseload in acute care, skilled facilities, acute rehab, and outpatient settings as appropriate.
  2. Participate in student education for part-time (ICE, OT level I) or full-time students.
  3. Assist in orientation and preceptorship of new or rotating staff.
  4. Complete educational, teaching, and professional activities relevant to the position.
  5. Plan, organize, and deliver treatment programs to aid clients/patients in recovery and rehabilitation.
  6. Utilize tools and techniques to administer safe therapeutic interventions and training for patients and families.
  7. Record and document patients' health progress, including daily treatments, progress notes, evaluations, and discharge summaries.
  8. Perform other duties as assigned.

Education:

Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program.

Experience:

Minimum of 2 years of clinical practice required.

Licenses and Certifications:

  • Valid state license in Physical Therapy.
  • CPR certification.
