Job summary

Northwell Health in Danbury is seeking a Physical Therapist for the Danbury Outpatient location. This full-time position includes comprehensive patient evaluations, treatment planning, and collaboration with a care team to improve functional outcomes.

The role requires CT State Physical Therapy License, CPR, and a bachelor’s degree with at least one year of experience; FIM certification is a plus. A $20,000 sign-on bonus is offered.

Qualifications

• Connecticut State Physical Therapy License required.
• FIM certification in Physical Therapy rehab settings preferred.
• CPR Certification required.
• Bachelor’s degree and minimum one year of experience in physical therapy.

Responsibilities

• Performs therapy evaluations, treatment planning and implementation per department guidelines; participates in rounds for care coordination.
• Provides patient and family education and training.
• Maintains documentation per guidelines; participates in performance improvement activities.
• Initiates discharge planning and recommends equipment to meet patient needs.
• Attends continuing education (20 hours/year) and provides staff inservice education (2 hours/year).
• Mentors lower-level staff and assists with orientation and delegation to support personnel.
• Fulfills compliance responsibilities and performs other duties as assigned.
