Job summary

Endeavor Health is seeking a Physical Therapist to provide direct PT services at our Plainfield, IL location. You will perform evaluations, create tailored treatment plans, and help patients achieve functional goals in a clinic that operates Monday–Thursday 7am–7pm and Friday 7am–4pm with one evening until 7pm.

Candidates should hold a Bachelor's in Physical Therapy, with preference for MS/PhD, and must have IL licensure and CPR/BLS certification.

Qualifications

  • Uses excellent communication skills to educate patients and engage them in their own care.
  • Bachelor’s Degree in Physical Therapy is required.
  • Master of Science or Doctorate in Physical Therapy is preferred.
  • Current State of Illinois Physical Therapist licensure is required.

Responsibilities

  • Provides direct physical therapy services to patients and develops individualized treatment plans.
  • Evaluates patient status and implements programs to correct movement dysfunction.
  • Maintains knowledge of current PT techniques and equipment.

Hourly Pay Range

$38.67 - $59.94 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights
  • Position: Physical Therapist
  • Location: 24600 W 127thh Street, Plainfield IL
  • Additional Location: Woodridge Outpatient site
  • Full-Time, 40 hours weekly
  • Hours: Clinic operation hours are Monday through Thursday, 7am to 7pm and Friday 7am to 4pm. One evening until 7pm required
  • Weekends: Two 4-hour Saturday shifts (8am to 12pm) per year at our Woodridge Outpatient location

While our primary population of patients is orthopedic physical therapy, we also offer Occupational and Speech services. We offer specialties in hand, neuro, vestibular, pelvic health, pediatrics, and lymphedema.

What You Will Do

Provides direct physical therapy services to patients. Demonstrates an appropriate level of clinical skill, as evidenced by the completion of comprehensive and insightful evaluations, and the development of individualized and effective treatment plans designed to achieve precise, functional goals. Evaluates patient’s physical and functional status and plans and implements therapeutic programs that include prevention, identification, correction and alleviation of acute or prolonged movement dysfunction of an anatomic or physiologic origin. Uses excellent communication skills to educate patients and engage them in their own care. Stays updated regarding developments, new techniques, and equipment in the field of physical therapy. Maintains a well-rounded knowledge base, and clinical reasoning skills to consistently achieve expected clinical outcomes.

What You Will Need
  • Bachelor’s Degree in Physical Therapy
  • Preferred: Master of Science or Doctorate in Physical Therapy
  • Current State of Illinois Physical Therapist licensure
  • A valid driver’s license is required if the incumbent is selected to perform related duties at an offsite location. If the incumbent uses their personal vehicle, the incumbent must maintain automobile liability coverage as required by law and evidence of such coverage may be requested.
  • American Heart Association BLS CPR certification
  • For Physical Therapist II level - minimum of 2 years work experience required
  • For Physical Therapist I level - open to 0 to 2 years' experience
Required Without License
  • Notification from Continental Testing Services advising of receipt and approval of application for licensure (if first-time test taker).
  • “Authorization to Test” (ATT) letter from Federation of State Boards of Physical Therapy
  • Confirmation of Computer-Based Exam
  • State of Illinois Physical Therapy license within 90 days of hire
  • Current CPR Certification issued by the American Heart Association BLS provider
