Responsibilities include preparing and maintaining accurate patient records, including evaluations, progress notes, discharge summaries, and billing documentation.

Qualifications:

• Licensed or eligible for licensure as a Physical Therapist in Illinois.
• Graduation from an accredited physical therapy program approved by the Committee on Allied Health Education and Accreditation of The Medical Association, the American Physical Therapy Association, or their educational equivalents.
• CPR certification required.
