Aspirus At Home in Portage, WI, is seeking a PHYSICAL THERAPIST HOURLY to join our field team!

• The Physical Therapist provides physical therapy to adult and geriatric patients.
• Evaluates, establishes treatment plans and goals, and supervises or administers treatment modalities tailored to patient needs based on age and diagnosis.
• Incorporates patient and family education into care.
• Provides documentation for all services rendered.

HOURS: Full Time / 80 hours every two weeks, day shift

Experience/Qualifications

• Knowledge of physical therapy standards and practices, typically acquired through a minimum of a bachelor’s degree in physical therapy from an APTA-accredited program.
• Current Wisconsin licensure to practice Physical Therapy is required.
• Current BLS certification or obtained within 90 days of hire.
• Valid Driver’s License required.
• Automobile liability insurance with coverage of $100,000/$300,000 or a $300,000 combined single-limit liability is required.
