Job summary

At Home Nursing Care, a boutique home health agency in San Marcos, CA, is seeking a part-time Physical Therapist to launch an Orthopedic Recovery at Home program. The role involves coordinating rehabilitative care, patient visits, and family education in North Coastal and North Inland San Diego County areas.

The ideal candidate has a PT degree, CA license, CPR, and 2 years of PT experience, ideally in community/home health. Flexible days and hours with potential for full-time growth are offered.

Qualifications

• Requires a degree in physical therapy (BPT/MPT).
• Licensed to practice as a PT in California (PT Board).
• Two years of PT experience preferred, home health experience a plus.
• Good verbal and written communication and organizational skills.
• Maintains CPR certification.
• Must have a valid driver's license and reliable vehicle.

Responsibilities

• Deliver physical therapy services per physician's plan of care.
• Follow company policies and licensing standards.
• Initiate therapy programs and educate patients and caregivers.
• Participate in discharge planning for patients.
• Prepare progress summaries as directed by policy.
• Provide PT consultations to families when indicated.
• Participate in peer consultation.
• Supervise PT assistants per policy and state rules.
• Occasional nights/weekends may be required to see clients.
