BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time position providing home health visits. This role services adult and geriatric clients across Lancaster and Lebanon County territories.

As a home care Physical Therapist, you will be a key member of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to stay at home.

Responsibilities include:

• Making home visits to clients in designated areas.
• Conducting diagnostic tests such as mobility, joint range, transfer status, stability, gait, strength, endurance, and safety assessments.
• Implementing wound prevention and repair interventions with evidence-based dressings and topical agents.
• Developing and updating individualized care plans, including manual therapy, gait training, and balance exercises.
• Assessing and revising care plans, collaborating with nursing staff as needed.
• Educating clients and families on rehabilitative activities to promote health and independence.
• Documenting observations and interventions accurately using electronic devices.

Qualifications:

• Current Pennsylvania Physical Therapist (PT) license.
• Graduation from an American Physical Therapy Association-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with electronic medical records preferred.
