Job description

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for full-time, part-time, or per diem roles providing home health visits in our senior living office located in Chester County.

This office serves adult and geriatric clients on a per-visit basis.

Applicants must have at least one year of clinical experience as a licensed Physical Therapist. As a home care PT, you will be a vital part of a multidisciplinary team delivering rehabilitative and skilled nursing care, enabling clients to remain at home.

Responsibilities include:

  • Making home visits to clients in designated areas.
  • Performing diagnostic tests and measurements related to mobility, strength, balance, and safety.
  • Implementing wound care interventions using evidence-based dressings and topical agents.
  • Developing and updating individualized care plans, including manual therapy, gait training, and balance exercises.
  • Assessing and revising care plans, collaborating with nursing and other team members.
  • Educating clients and their families on rehabilitative activities to promote health and independence.
  • Documenting observations and interventions accurately using electronic tablets.

Qualifications:

  • Current Pennsylvania PT license.
  • Minimum of one year recent experience as a PT.
  • Graduation from an accredited program by the American Physical Therapy Association or equivalent.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Proficiency with computers; EMR experience preferred.
