BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT, for a full-time opportunity performing home health visits throughout our senior living office in the East Side of Cuyahoga County and Portage County.

This office services adult and geriatric clients on a per-visit basis.

One year of prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to receive necessary medical care to remain at home.

Responsibilities for a PT - Physical Therapist include:

1. Make home visits to clients in designated geographic areas.
2. Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, ambulation patterns and appearance, muscle strength and endurance, balance testing, and safety assessments.
3. Implement integumentary protective interventions to prevent wound formation and promote healing, utilizing evidence-based dressings and topical agents.
4. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
5. Continuously assess and revise the care plan, collaborating with nursing staff as needed.
6. Educate clients, family members, or representatives on rehabilitative care and activities to promote health, safety, and independence.
7. Document observations, interventions, and evaluations accurately using a state-of-the-art touch pad tablet.

Qualifications for a PT - Physical Therapist include:

1. Current Ohio Physical Therapist - PT license.
2. Minimum of one year recent experience as a Physical Therapist - PT.
3. Graduation from an American Physical Therapy Association-approved program, evidenced by transcript or diploma.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal, organizational, and problem-solving skills.
6. Solid computer skills; prior EMR experience preferred.
