BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for full-time, part-time, or per diem roles providing home health visits in West Philadelphia and South Philadelphia.

This position services adult and geriatric clients on a per-visit basis.

As a home care Physical Therapist, you will be a key member of a multidisciplinary healthcare team, delivering rehabilitative care and skilled nursing to enable clients to remain at home.

Responsibilities include:

1. Making home visits to clients in designated areas.
2. Performing diagnostic tests such as mobility, strength, balance, and safety assessments.
3. Implementing wound care interventions using evidence-based dressings and topical agents.
4. Developing and revising individualized care plans, including manual therapies, gait training, and balance exercises.
5. Assessing and updating care plans regularly and collaborating with nursing staff.
6. Educating clients and family members on rehabilitative activities to promote health and independence.
7. Documenting observations and interventions accurately using electronic devices.

Qualifications:

• Current Pennsylvania Physical Therapist (PT) license.
• Graduation from an American Physical Therapy Association-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency in computer skills; experience with electronic medical records preferred.
