BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time position providing home health visits in Baltimore, MD. This role involves servicing adult and geriatric clients across territories in the Baltimore City area.

Applicants must have at least one year of clinical experience as a licensed Physical Therapist. As a home care PT, you will be part of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities include:

1. Making home visits to clients in designated areas.
2. Performing diagnostic tests and measurements, including mobility, strength, balance, and safety assessments.
3. Developing and implementing individualized care plans with manual therapies, gait training, and other interventions.
4. Assessing and revising care plans regularly and collaborating with nursing staff.
5. Educating clients and families on rehabilitative activities to promote health and independence.
6. Documenting observations and interventions accurately using electronic tablets.

Qualifications:

1. Current MD Physical Therapist license.
2. Graduation from an accredited program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with computers; experience with electronic medical records preferred.
