Job Opportunity: Physical Therapist (PT) at BAYADA Home Health Care

Up to a $10,000 Sign-On Bonus for Full-Time

BAYADA Home Health Care is seeking an experienced Physical Therapist, PT for a full-time position providing home health visits throughout Berks County. As a home care Physical Therapist, you will be a vital part of a multidisciplinary healthcare team delivering rehabilitative care and skilled nursing to clients, enabling them to stay at home.

Responsibilities:

1. Make home visits to clients within designated geographic areas.
2. Perform diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Implement protective interventions to prevent wounds and promote healing, utilizing evidence-based dressings and topical agents.
4. Develop and execute individualized care plans, including manual exercises, gait training, and balance interventions.
5. Continuously assess and update care plans, collaborating with nursing staff as needed.
6. Educate clients and their families on rehabilitative activities to promote health, safety, and independence.
7. Document observations, interventions, and evaluations accurately using a state-of-the-art touchpad tablet.

Qualifications:

1. Current Pennsylvania Physical Therapist - PT license.
2. Graduation from an American Physical Therapy Association-approved program or equivalent.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with computers; experience with electronic medical records (EMR) preferred.
