Job Opportunity: Physical Therapist (PT) at BAYADA Home Health Care

Position Type: Full Time, Part Time, or Per Diem
Location: Serving adult and geriatric clients in New Castle County

Overview: BAYADA Home Health Care is seeking an experienced Physical Therapist to provide rehabilitative care through home health visits. The role involves working as part of a multidisciplinary team to enable clients to receive necessary medical care at home.

Responsibilities include:

1. Making home visits to clients within designated territories.
2. Performing diagnostic tests and measurements related to mobility, strength, balance, and safety.
3. Implementing wound care interventions using evidence-based dressings and topical agents.
4. Developing and adjusting personalized care plans, including manual therapy, gait training, and balance exercises.
5. Assessing and revising care plans in collaboration with nursing staff.
6. Educating clients and their families on rehabilitative activities to promote health and independence.
7. Documenting observations and interventions accurately using electronic medical records.

Qualifications:

• Current Delaware Physical Therapist (PT) license.
• At least one year of recent clinical experience as a PT.
• Graduation from an accredited physical therapy program.
• Ability to work independently and effectively manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computer systems; EMR experience preferred.
