Job description

We are currently seeking compassionate, motivated, and experienced home health Physical Therapists.

Our goal is to provide QUALITY services to our patients.

Essential Responsibilities:

1. Evaluate patients and interpret evaluation findings to determine the nature and extent of dysfunction, including needs for adaptive equipment and Activities of Daily Living.
2. Establish treatment goals and plan treatments to achieve these goals, including adaptive equipment as appropriate.
3. Initiate discharge planning and implement the plan of care through direct treatment, delegation to supportive personnel, and coordination with other healthcare providers. Provide education to patients and families. Reassess clinical signs and symptoms regularly to evaluate treatment effectiveness and adjust goals as needed.
4. Assess the need for adaptive equipment for Activities of Daily Living based on clinical guidelines. Coordinate assessments with certified vendors. Reassess equipment as necessary and discharge patients when maximum benefit has been achieved.
5. Supervise the work of Home Health Aides in accordance with regulations and agency policies. Demonstrate ongoing professional development.
6. Provide accurate and timely documentation of patient management, including assessments for adaptive equipment, in accordance with agency policy.
