Physical Therapist ‐ Home Health ‐ (PT ‐ HH)

Start Date: 7/21/2025 | Available Shifts: 12 D | Pay Rate: $1984.00 ‐ $2063.00

Job Overview:

We are seeking a Physical Therapist for home health services, to start ASAP. Responsibilities include performing comprehensive physical therapy assessments considering physiological, psychosocial, environmental factors, and health history. Develop and implement plans of care aimed at promoting function and safety. Collaborate with staff to develop agency policies and programs related to patient care.

Qualifications:

• Bachelor or Master of Science in Physical Therapy from an accredited institution by the American Medical Association and the American Physical Therapy Association.
• Current PT license in Massachusetts.
• At least 1 year of clinical experience.
• CPR certification (copy required).
• Computer skills preferred.

Additional Requirements:

• PT license in MA.
• Valid BLS certification.
• SSN and DOB required.
• Work history should not have gaps exceeding 30 days.
• At least one professional reference.
