Job description

BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a full-time opportunity performing home health visits in Nashua, Manchester, and surrounding towns in NH. This office services adult and geriatric clients on a per-visit basis throughout the Manchester/Nashua area.

One year of prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to remain at home.

Responsibilities include:

1. Make home visits to clients in designated geographic territories.
2. Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
3. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
4. Assess and revise care plans regularly, collaborating with nursing staff as appropriate.
5. Educate clients and their families on rehabilitative care and activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using a touchpad tablet.

Qualifications:

• Current New Hampshire Physical Therapist (PT) license.
• Minimum of one year recent experience as a PT.
• Graduation from an American Physical Therapy Association-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; experience with electronic medical records (EMR) preferred.
