Position Summary

The Home Health Physical Therapist - PPV is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation. The therapist upholds the Code of Ethical Conduct and fosters positive working relationships within the company and with external stakeholders. They provide coordinated care to patients of all ages, planning, implementing, and evaluating care plans to restore or maintain well-being. The therapist organizes and conducts physical therapy treatments based on medical referrals and evaluations, collaborating with the interdisciplinary team (IDT).

Position Specific Responsibilities

  • Provides care utilizing infection control measures that protect both staff and patients (OSHA).
  • Demonstrates commitment to professional growth by attending in-services and participating in programs.
  • Accepts clinical assignments appropriate to their education and competence.
  • Monitors cases for compliance with third-party payor requirements.
  • Communicates effectively with patients, families, team members, and other healthcare professionals, providing referrals as needed.
  • Performs initial and ongoing assessments, including OASIS assessments, to identify patient needs and determine the appropriateness for home health.
  • Oversees other team members in patient care, ensuring needs and desires are met.
  • Prepares and revises the Plan of Care in consultation with physicians and team members, documenting progress and assessments.
  • Provides treatment for neuromuscular, neurological, and orthopedic disorders based on referrals and assessments.
  • Instructs patients and families on transfer techniques and use of equipment like walkers, wheelchairs, and canes.
  • Performs other duties as assigned.

Education and Experience

  • Bachelor of Science in Physical Therapy required.
  • Two years of experience in an acute care or rehabilitation setting preferred.

Skills

  • Mathematical skills: Ability to perform calculations involving measurements, rates, ratios, and percentages.
  • Language skills: Ability to read, interpret, and communicate effectively in business and clinical contexts.
  • Other skills: Proficiency in English, articulating and embracing hospice philosophy.

Certifications, Licenses, and Registrations

  • Active and unencumbered Physical Therapist license in the state of employment.
  • Current CPR certification.

Physical Demands and Work Environment

Requires standing, walking, manual dexterity, and the ability to lift up to 50 pounds. Visual acuity and exposure to bodily fluids and infectious diseases are expected. Adaptations are available for team members with disabilities.
