Hutchinson Regional Healthcare System is seeking a Physical Therapist for Home Health (PRN) to administer neuro/musculoskeletal rehabilitation under physician orders. The role serves patients across the lifespan with a focus on safe, effective outcomes.

Physical Therapist - Home Health - PRN ESSENTIAL FUNCTIONS:

• Performes standardized and non-standardized assessments that are comprehensive, age appropriate and specific to the patient's need. Demonstrates knowledge of the illness, injury or disability of the patient being served.
• Formulates a plan of care from the comprehensive assessment that includes a diagnosis and measurable goals and objectives.
• Maximizes the patient's abilities in a safe, effective and appropriate manner, prioritizing patient needs and utilizing available resources.
• Clinically competent in performing evaluations, utilizing assessment tools and intervention programs and use of modalities that determine the effectiveness of actions in obtaining goals and objectives.
• Insures functional outcomes and continuity of care through discharge planning.
• Assesses patient/family preferences, goals and needs and implements appropriate measures to meet these.
• Performs patient care and completes documentation which is timely, accurate, and demonstrates medical necessity.
• Demonstrates awareness and sensitivity to patient/family rights.
• Meets productivity standards as set per clinical area.
• Serves a population newborn through geriatric.
• Abides by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI).

MINIMUM KNOWLEDGE AND SKILLS REQUIRED :

• Knowledge of Physical therapy principles.
• Demonstrated ability to communicate effectively.

MINIMUM EDUCATION AND EXPERIENCE REQUIRED:

New graduates possess a doctorate in Physical Therapy from an accredited college. Experienced Physical Therapist will need a Bachelor's or Master's depending on the graduation date.

PREFERRED EDUCATION AND EXPERIENCE:

Several years' experience in a medical center or rehab facility.

REQUIRED LICENSE/REGISTRATION/CERTIFICATION:

• Current licensure in Physical Therapy through the Kansas Board of Healing Arts.
• BLS - BLS is required within three months of hire
• Kansas State Driver's License
• Auto Insurance equal to or greater than the Kansas insurance requirements.
