Job summary

Elite Home Health in Florida seeks a dedicated Physical Therapist (PT) to provide skilled therapy in patients' homes across Leesburg and surrounding areas. You will assess, plan, and implement individualized treatment programs tailored to mobility and independence goals.

The role requires a Florida PT license, CPR certification, and 1+ year of PT experience preferred, with strong communication to collaborate with physicians and care teams. Travel and home-based visits form part of this position.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Florida PT license in good standing.
• CPR certification.
• Background and compliance requirements must be met.

Responsibilities

• Perform physical therapy evaluations and develop individualized treatment plans.
• Provide skilled therapy, including therapeutic exercises, gait training, balance training, and mobility interventions.
• Educate patients and caregivers on exercises, safety, fall prevention, and assistive devices.
• Monitor patient progress and adjust treatment plans as needed.
• Complete timely and accurate clinical documentation.
• Communicate with physicians, nurses, and the interdisciplinary care team.
• Supervise PTAs as required.

We are currently hiring a Physical Therapist (PT) to provide skilled therapy services to patients in their homes throughout Leesburg and surrounding areas.
