Job summary

Elite Home Health Group INC is hiring a Licensed Physical Therapist (PT) for home health in Gainesville, FL. The position requires an active Florida PT license and offers PRN opportunities throughout Leesburg and surrounding areas.

You will evaluate patients at home, create individualized treatment plans, provide therapy, educate families, and coordinate with the care team to maintain high-quality, compassionate care.

Qualifications

• Active Florida PT license in good standing.
• Minimum 1 year PT experience preferred.
• Current CPR certification.

Responsibilities

• Perform physical therapy evaluations and develop individualized treatment plans.
• Provide skilled therapy including exercises, gait training, balance training, and mobility interventions.
• Educate patients and caregivers on exercises, safety, fall prevention, and assistive devices.
• Monitor patient progress and adjust treatment plans as needed.
• Complete timely and accurate clinical documentation.
• Communicate with physicians, nurses, and the interdisciplinary care team.
• Supervise PTAs as required.
