Job summary

VitalCaring is seeking a Home Health Physical Therapist to join our compassionate team. You will deliver patient-centered, at-home care, assess functional status, build individualized plans, and collaborate with physicians to improve mobility and independence.

This highly autonomous, relationship-driven role offers flexible scheduling and opportunities for growth as you help patients recover in the comfort of their homes.

Qualifications

• Graduate from an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state where you will practice.
• 1–2 years of clinical experience; home health preferred.
• Strong clinical assessment and patient education abilities.
• CPR certification and valid driver's license.

Responsibilities

• Deliver patient-centered care and home-based physical therapy.
• Evaluate functional status, mobility, and rehabilitation potential.
• Develop and implement individual care plans with physicians.
• Educate patients and caregivers on exercises, safety, and plans.
• Document care and progress in the EMR and participate in team meetings.
• Supervise PTAs in accordance with state guidelines.

What You'll Do

Deliver Patient-Centered Care

• Evaluate and assess patients' functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies

Drive Outcomes & Recovery

• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional patient needs and collaborate with interdisciplinary teams
• Partner closely with nurses, therapists, and physicians to ensure coordinated care
• Document patient care and progress accurately and timely within the EMR
• Participate in case conferences and team meetings

Lead & Support

• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff

What You Bring

• Graduate of an accredited Physical Therapy program (DPT or Master's)
• Current PT license in the state of practice
• Minimum 1-2 years of experience (home health experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver's license, reliable transportation, and auto insurance
