Job description

Summary

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment, the therapist develops a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

Annual Compensation: $100,000 - $110,000 (Based on full-time schedule).

Bonus: $5,000.00

Responsibilities

  1. Evaluate the patient's functional status, body systems, and therapy needs; consult with physicians for care planning.
  2. Provide services within the scope of practice as defined by state laws, in accordance with the plan of care, and coordinate with the healthcare team.
  3. Observe and report patient responses and condition changes to supervisors or physicians.
  4. Treat patients to relieve pain, restore or develop function, and maintain maximum functioning using accepted standards and evidence-based techniques.
  5. Educate the healthcare team, patients, and families about the care plan, limitations, and progress.
  6. Visit patients within the service area and nearby agencies as needed.
  7. Complete progress notes for each visit and submit them per policy.
  8. Conduct evaluations, develop the PT plan of care, and report outcomes within 24 hours.
  9. Participate in staff meetings and committees as required.
  10. Supervise home health aides and PTAs according to policy and state regulations.
  11. Maintain schedule flexibility, participate in weekend rotations, and be available on observed holidays.
  12. Prepare discharge summaries and condition reports every 60 days.
  13. Provide in-service education as requested.
  14. Adhere to therapy standards of practice and maintain up-to-date clinical knowledge and licensure.
  15. Perform all other duties as assigned.

Education and Experience

Licensure Requirements:

  • Current Colorado Physical Therapy license.
  • Current CPR certification.
  • Valid driver’s license, vehicle insurance, and access to a dependable vehicle.
