What You’ll Do:

• Conduct comprehensive physical therapy assessments in the patient’s home

• Develop and implement individualized care plans that reflect patient goals and physician directives

• Provide therapeutic exercises, gait training, and home safety evaluations

• Educate patients and families on injury prevention and proper mobility techniques

• Coordinate care with other disciplines including nursing, occupational therapy, and speech therapy

• Document timely and accurate clinical notes in compliance with all regulations

Qualifications

What We’re Looking For:

• Active Physical Therapist license in the state of Michigan

• Minimum of one (1) year of PT experience, preferably in a home health setting

• Strong communication skills and a patient-centered mindset

• Reliable transportation, valid driver’s license, and current auto insurance

• Ability to travel within assigned coverage areas

• Passion for empowering patients to achieve their best quality of life

Compensation:

• Per Visit Rate: $65 – $75 (based on experience and location)
