Henry Ford Home Health Care Henry Ford Home Health Care is a comprehensive network of home health products and services that focuses on the complex needs of patients and caregivers. Available programs include chronic care management, joint replacement care, specialty infusion and transplant care, wound, ostomy and continence nursing, and palliative care to name a few. Henry Ford Home Health Care’s services are provided by qualified registered nurses, licensed practical nurses, certified wound, ostomy and continence nurses, physical therapists, occupational therapists, speech - language pathologists, medical social workers, dietitians, and home health aides.
GENERAL SUMMARY:
Under general supervision of the therapy supervisor implements medically prescribed physical therapy treatment programs, (e.g., heat, water, exercise, etc.) directed towards relieving pain and/or restoring physical functions to rehabilitative clients.
EDUCATION AND EXPERIENCE:
·Accredited Bachelor’s Degree in Physical Therapy required to relieve pain and/or restore a physiological function.
LICENSE/CERTIFACTIONS:
·Requires current Physical Therapist licensure in the State of Michigan.
·Must possess a current State of Michigan driver's license and a satisfactory driving record.
OTHER SKILLS REQUIRED:
·Must be able to lift and position clients and perform treatment modalities.
·High level of interpersonal skill is required to effectively interface with clients and physicians.
·Must have reliable transportation and a telephone.
