Job summary

Extended Home Care is seeking a compassionate Physical Therapist (PT) for full-time home visits in the Bronx and surrounding areas. This role offers flexibility, autonomy, and strong clinical support within a growing home health organization.

Salary ranges from $110,000 to $120,000 annually, plus a $500 monthly travel stipend. Candidates should have a PT degree, current licensure, CPR certification, and a valid driver's license to serve in clients’ homes.

Qualifications

• Must hold a Physical Therapy degree from an accredited program.
• Current state license to practice physical therapy.
• CPR certification required.
• Valid driver's license and insured vehicle.

Responsibilities

• Evaluates home environment to determine therapeutic needs.
• Assesses muscle strength, mobility, gait and range of motion for rehab potential.
• Directs physical therapy treatment and progress.
• Instructs patients and families in use and care of therapeutic devices.
• Reports patient progress and coordinates with physician as needed.
• Supervises assistants and guides home health aides per policy.

Full-Time Salary: $110,000 - $120,000 + $500 Monthly Travel Stipend

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

• Evaluateshomeenvironment.
• Assesses for muscle strength, mobility, gait, ROM-potential for rehab.
• Directing physical therapy treatment.
• Instructing patients and families/caregivers in the use and care of therapeutic appliances.
• Determining priority needs for physical therapy
• Reporting to physician patient's reaction to treatment or changes in condition.
• Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
• May train patient in the use of prosthetic device.
• Identifies patient and family/caregiver needs for other home health services and refers as necessary.
• Prepares and submits clinical and progress summaries based on the attainment of goals.
• Participates in discharge planning for patient.
• Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy.
• Provides physical therapy consultation to home families/caregivers when indicated.
• Provides in-service education programs for nursing organization personnel as needed.
• Participates in peer consultation process.
• Supervises Physical Therapy Assistants according to organization policy and state regulations.
• When therapy is the only skilled service, instructs. supervises and evaluates home health aide care every two (2) weeks.

Position Requirements:

• Possesses a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization.
• Licensedtopracticeasphysical therapist within theState,ifapplicable.
• Two years ofappropriateexperienceas aphysicaltherapist.Community/homehealthexperienceispreferred.
• Demonstrates goodverbalandwrittencommunication and organization skills.
• Possessesandmaintainscurrent CPRcertification.
• Mustbe alicenseddriverwithan automobile thatisinsuredin accordance with state ororganization requirementsand is ingoodworkingorder.
