Job summary

Alpine Therapy Services LLC is seeking a Home Health Physical Therapist to evaluate and treat patients in Boulder, Longmont, and surrounding areas. PRN, part-time, or full-time positions available.

Responsibilities include initial and ongoing evaluations, developing and adjusting treatment plans, and collaborating with patients to improve mobility and function in home health settings. Licensure in Colorado and CPR certification are required.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Current licensure to practice Physical Therapy in Colorado required or provisional license.
• Excellent interpersonal skills and ability to engage patients.

Responsibilities

• Perform initial and ongoing physical therapy evaluations in community settings according to APTA standards and state requirements.
• Implement and adjust plans of treatment on a schedule based on patient progress.
• Provide therapy and restoration services directly or via supervision/teaching to others.
• Evaluate mobility, strength and function; set realistic, patient-centered goals.
• Participate in quality assessment and professional development.

As a Physical Therapist, what you’ll do:

Perform initial and ongoing physical therapy evaluations in the community or place of residence according to APTA standards of practice as well as state standards.

Implement Plan of Treatment on a consistent schedule and adjust this schedule and plan according to patient progress.

Provide therapy and treatment that is supportive and restorative to life and well-being either directly to the patient or indirectly through consultation with, delegation to, supervision of, or teaching of others.

Evaluate mobility, strength and current level of patient function, while setting goals to improve in these areas that are scaled to patient abilities and that are realistically attainable.

Participate as assigned in quality assessment performance improvement initiatives, and maintains professional knowledge and experience through ongoing education and training

As a Physical Therapist, what you’ll need:

Bachelor's degree in Physical Therapy required. Preferred: Master's or Doctorate degree in Physical Therapy.

Current licensure (or provisional license as allowable by the State Physical Therapy Practice Act) with Colorado Department of Regulatory Agencies to practice Physical Therapy required

Excellent interpersonal skill and ability
