Help Patients Heal at Home - On a Schedule That Works for You

We're looking for a licensed and experienced Physical Therapist to join our team on a Baylor shift: 10 hour days on Saturday, Sunday + either Thursday or Friday; work three days and enjoy the flexibility of full-time pay.

What You'll Do
- Conduct in-home physical therapy evaluations and develop individualized treatment plans
- Perform OASIS assessments and ensure timely documentation
- Supervise and coordinate care with PTAs and other clinical team members
- Deliver evidence-based treatments to help patients regain mobility, strength, and independence
- Educate patients, families, and caregivers on treatment plans and safe practices
- Collaborate in case conferences and patient care planning
- Maintain accurate clinical notes, follow Medicare and compliance guidelines
- Support transitions of care and make appropriate referrals as needed
- Ensure ongoing communication with physicians and agency team
What You'll Need
- Bachelor's, Master's, or DPT from an accredited Physical Therapy program
- Minimum 2 years of clinical experience (home health preferred)
- Strong documentation, time management, and interpersonal skills
- Proficiency in using tablets and EMR systems
- Valid driver's license and reliable transportation (local travel required)

Compensation:Th pay range listed represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
