We're looking for a licensed and experienced Physical Therapist to join our team on a Baylor shift: 10 hour days on Saturday, Sunday + either Thursday or Friday; work three days and enjoy the flexibility of full-time pay.

What You'll Do

• Conduct in-home physical therapy evaluations and develop individualized treatment plans
• Perform OASIS assessments and ensure timely documentation
• Supervise and coordinate care with PTAs and other clinical team members
• Deliver evidence-based treatments to help patients regain mobility, strength, and independence
• Educate patients, families, and caregivers on treatment plans and safe practices
• Collaborate in case conferences and patient care planning
• Maintain accurate clinical notes, follow Medicare and compliance guidelines
• Support transitions of care and make appropriate referrals as needed
• Ensure ongoing communication with physicians and agency team

What You'll Need

• Bachelor's, Master's, or DPT from an accredited Physical Therapy program

• Strong documentation, time management, and interpersonal skills
