As a therapist at CenterWell Home Health, you'll play a vital role in helping patients regain strength, mobility, and independence—all from the comfort of their homes. By delivering personalized care focused on rehabilitation and functional improvement, you'll empower individuals to overcome physical limitations, perform everyday activities with confidence, and enjoy a better quality of life.

Responsibilities include:

• Planning and administering prescribed skilled physical therapy treatments for patients with various injuries, illnesses, and disabilities to maximize physical function.
• Testing and screening patients' physical strengths to assist physicians in evaluating their level of function and documenting findings to develop treatment programs and measurable objectives.
• Developing and implementing conditioning and rehabilitation programs aligned with physicians' Plans of Treatment and patient goals, adjusting treatments as needed.
• Collaborating with physicians and clinical team members to gather additional patient information and revise therapy plans accordingly.
• Providing instructions and care plans to Physical Therapy Assistants and Home Health Aides, supervising their performance, and monitoring documentation accuracy.
• Documenting patient care observations, interventions, and evaluations thoroughly and promptly, ensuring compliance with orders and policies.
• Reporting patient progress to physicians, managers, patients, and families, and submitting evaluations, treatment plans, progress reports, and discharge summaries.

Required Experience and Skills:

• Degree from an accredited Physical Therapy Program (approved by the APTA).
• Preferably, at least one year of physical therapy experience.
• Valid and unrestricted Physical Therapy license.
• Current CPR certification.
• Strong organizational and communication skills.

Pay Range:

• $64.00 - $89.00 per visit/unit.
• $93,900 - $129,300 annually, based on full-time employment and other factors.
