Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on assessments, the therapist develops treatment plans, performs interventions to improve patient well-being, and evaluates progress.

  • Evaluate the patient's functional status and physical therapy needs; assist physicians in developing the therapy care plan.
  • Monitor and report patient responses to treatment and any condition changes to the supervising nurse or physician.
  • Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to accepted standards.
  • Educate and inform the healthcare team, patients, and families about the care plan, functional limitations, and progress toward goals.
Education and Experience

Licensure Requirements

  • Current Physical Therapy licensure in the state of practice
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to a reliable vehicle

  • AL: No additional specific requirements.
