Sutter Health's SCAH-Sutter Care at Home - Bay area seeks a Physical Therapist to assess and guide rehabilitative programs across Campbell, Saratoga, Los Gatos, San Jose, Morgan Hill, Gilroy and nearby communities.

You will develop clinical protocols, educate staff, and ensure regulatory compliance while delivering high-quality patient care in a home health setting.

Qualifications

• Graduate from an accredited physical therapy program.
• PT-Physical Therapist licensed or eligible.
• BLS Healthcare Provider certification.

Position Overview

Primary coverage area Campbell, Saratoga, Los Gatos, San Jose, Morgan Hill, Gilroy and surrounding.

Responsible for assessing, planning, organizing, and participating in rehabilitative programs that improve patient mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury. Demonstrates advanced knowledge and practice level competencies and is a recognized clinical or workflow subject matter expert. Responsible for developing key workflows and clinical protocols or guidelines for department. Serves as a resource in the education, training and mentoring of other staff and as a clinical instructor for assigned students. May coordinate staff education and student programs. Adheres to all local/state/federal regulations, codes, policies and procedures to ensure privacy and safety while delivering optimal patient care.

• Other: Graduate of an accredited physical therapy school

Pay Range is $74.17 to $103.84 / hour

The posted pay range is based on the position's primary work location. Candidates who reside and work outside of that location may be subject to a different pay range based on their work location. Candidate starting salaries consider multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs.
