Pathways Healthcare is seeking a highly qualified Physical Therapist to join our team as a per diem clinician in the Londonderry, NH area. You will evaluate patients, develop rehabilitation plans, and collaborate with families and staff to ensure a cohesive approach to care.

The role emphasizes in-home and community health services, with opportunities to lead patient education and participate in discharge planning. A valid PT license, BLS CPR, and a reliable vehicle are required.

Qualifications

• Must hold a degree as a physical therapist from an accredited program.
• Minimum two years of relevant PT experience preferred, community/home health experience desirable.
• Excellent verbal and written communication skills.
• Valid driver’s license and own reliable vehicle.
• Current BLS CPR certification.

Responsibilities

• Provide PT services per physician’s plan of care; assess mobility, gait, ROM, and strength.
• Develop individualized PT programs and educate family/caregivers.
• Instruct on use of therapeutic appliances and prosthetic devices; train as needed.
• Participate in discharge planning and refer to home health services as required.
• Prepare progress summaries per organizational policy; engage in peer consultation.

About the Role:

We are seeking a highly qualified and compassionate Physical Therapist to join our dedicated team. As a Physical Therapist, you will have the opportunity to make a significant impact on the lives of our patients by providing exceptional care and support.

Position Details:

• Main Office Location: Londonderry, NH
• Service Area: Salem and surrounding areas.
• Job Type: Per Diem

Responsibilities:

• Provide physical therapy services to patients in accordance with a written physician’s plan of care and the state Physical Therapy Practice Act. This includes assessing and evaluating therapeutic/rehabilitative/functional status, evaluating the home environment, and determining the potential for rehabilitation, including muscle strength, mobility, gait, and range of motion.
• Initiate and develop individualized physical therapy programs for patients while also instructing other personnel and/or family/caregiver members involved in the patient’s care. Actively participate in case conferences to ensure a comprehensive and coordinated treatment approach.
• Instruct patients and their families/caregivers in the use and care of therapeutic appliances and, when necessary, train patients in the use of prosthetic devices. Additionally, provide in-service education programs for nursing organization personnel as needed.
• Participate in discharge planning for patients, identifying their ongoing needs and referring them to other home health services as required.
• Prepare and submit clinical progress summaries based on the achievement of goals, following the guidelines set forth by the Organization policy.
• Offer physical therapy assistant consultation to home families/caregivers when indicated, and actively engage in the peer consultation process to enhance the overall quality of care.

Qualifications:

• Possess a degree as a physical therapist approved by an accredited organization.
• A minimum of two years of relevant experience as a physical therapist (preferred), with priority given to those with community/home health experience is desirable.
• Demonstrate excellent verbal and written communication skills, as well as organizational abilities.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable, insured vehicle.
