Summary

In accordance with national standards of practice and within the guidelines and policies of MHCAH and the Maine State Board of Licensure, the Home Care PT provides reliable, evidence-based, patient-family-centered quality care through assessment, diagnosis, outcome identification, planning, implementation, documentation, and evaluation of care outcomes. We are a dynamic team of Home Healthcare professionals dedicated to delivering mission-aligned care that ensures our "communities are the healthiest in America".

Required Minimum Knowledge, Skills, and Abilities (KSAs)

1. Education: Graduation from an accredited Physical Therapy program.
2. License/Certifications: State of Maine licensure as a Physical Therapist. Valid driver's license with a vehicle covered by liability insurance, as per agency policy. Current certification in Basic Life Support for Healthcare providers.
3. Experience: Minimum one year of full-time equivalent experience in a supervised setting.
4. Interpersonal and communication skills to interact effectively with patients, families, and healthcare team members.
5. Analytical ability to identify changes in patients' conditions and initiate appropriate actions.
