Summary:

The Home Care PT supports MaineHealth Care at Home's (MHCAH) mission, vision, and values by exhibiting excellence, competence, collaboration, innovation, respect, patient and family-centered care, commitment to our community, and accountability. In accordance with national standards of practice and within the guidelines and policies of MHCAH and the Maine State Board of Licensure, the Home Care PT provides reliable, evidence-based, patient-family-centered quality care through assessment, diagnosis, outcome identification, planning, implementation, documentation, and evaluation of care effects.

Required Minimum Knowledge, Skills, and Abilities (KSAs):

• Education: Graduation from an accredited physical therapy program.
• License/Certifications: State of Maine licensure as a Physical Therapist. Valid driver’s license with vehicle for work, covered by agency liability insurance. Current Basic Life Support certification for Healthcare providers.
• Experience: Minimum one year of full-time equivalent supervised experience.
• Interpersonal and communication skills for effective interaction with patients, families, and healthcare team members.
• Analytical skills to identify changes in patients’ conditions and initiate appropriate actions.
