Responsibilities

As a Home Health Physical Therapist, you will:

• Perform physical therapy evaluations and treatments based on physician orders, including adaptive equipment recommendations
• Establish measurable goals and develop treatment plans to maximize patient independence and function
• Maintain communication among the treatment team, physician, patient, and family
• Document services thoroughly and timely for proper billing
• Supervise Physical Therapist Assistants and Aides in patient care per state practice act

Qualifications

• Graduate of a CAPTE-approved Physical Therapist Education program
• Current Physical Therapist licensure in Tennessee and CPR certification; CPR may be offered upon hire
• At least 2 years of experience in adult physical therapy; home health experience preferred
• Excellent communication, problem-solving, and basic math skills; competency in physical therapy skills
