Job summary

BAYADA Home Health Care is seeking a new graduate Physical Therapist to join our teams in Hawaii, providing mentorship and hands-on in-home care to adult and geriatric clients. You will evaluate function, develop personalized treatment plans, and deliver one-on-one therapy in patients' homes.

Our orientation and clinical support help new graduates bridge classroom and independent practice, with flexible scheduling, mileage reimbursement, and ongoing professional development opportunities.

Qualifications

• Graduate of an accredited Doctor of Physical Therapy (DPT) program.
• Licensed or license-eligible Physical Therapist in the state of practice.
• New graduates are strongly encouraged to apply.
• Excellent communication and interpersonal skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate and assess patients' functional status and mobility.
• Develop individualized treatment plans to maximize independence and safety.
• Provide skilled therapeutic interventions in patients' homes.
• Educate patients, families, and caregivers on exercises, mobility, fall prevention, and home safety.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
• Complete timely electronic documentation while maintaining the highest standards of patient care.
