Job summary

Amedisys, part of the Optum family, is hiring a Physical Therapist to provide skilled PT services in patients' homes to promote mobility, restore function, and improve safety and independence. The PT works with patients, caregivers and the care team to support recovery and prevent hospitalizations.

You will perform 30-day reexaminations, adjust interventions as needed, and participate in discharge planning.

Qualifications

• Licensed PT in the state you work in.
• Graduate from a CAPTE-accredited physical therapy program.
• 1+ year of physical therapy experience in home health care or equivalent.
• If <1 year, mentoring and/or residency approval may be required.

As the Physical Therapist (PT) you will be responsible for providing skilled physical therapy services to patients in their homes to promote mobility, restore function, improve strength and balance, and enhance overall safety and independence. The Physical Therapist works collaboratively with patients, caregivers, and an interdisciplinary care team to support recovery, prevent hospitalizations, and improve quality of life within the home setting.

Primary Responsibilities:

• Responsible for helping patients maintain, restore and improve movement, activity and function
• Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning
• Other duties as assigned

Required Qualifications:

• Current license to practice physical therapy specific to the state you're assigned to work in-
• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE)
• 1+ related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program
• Current CPR certification

The salary for this role will range from $80,864 to $121,296 annually based on full-time employment. We comply with all minimum wage laws as applicable.
