We are currently seeking a dedicated and passionate Physical Therapist to join our Home Health team. This is a full-time, permanent position that offers the unique opportunity to provide one-on-one, personalized care in the comfort of patients' homes. This role is perfect for someone who loves the idea of making a real difference in people's lives, and who thrives in a fast-paced, dynamic, and rewarding healthcare environment.
Responsibilities :
As a Home Health Physical Therapist, you will be responsible for :
1. Evaluating patients' functional status, physical therapy needs, and setting achievable goals to improve their quality of life.
2. Developing individualized treatment plans based on each patient's unique needs and goals, in collaboration with the multidisciplinary healthcare team.
3. Implementing and modifying the physical therapy treatment program to meet the patient's needs.
4. Performing home safety evaluations and patient education to ensure a safe and supportive environment.
5. Documenting patient's progress, response to therapy, and updating the plan of care as necessary using the Oasis Assessment system.
6. Coordinating with other healthcare professionals, caregivers, and family members to ensure continuity of care.
7. Providing training and education to family members and caregivers to support patients' therapy and rehabilitation goals.
8. Complying with all federal and state regulations, ethical standards, and policies related to home health care.
Qualifications :
To be considered for this exciting opportunity, the candidate must have :
1. A Degree in Physical Therapy (DPT) from an accredited program.
2. A current and valid Physical Therapist (PT) license in the state of practice.
3. A minimum of 2 years of experience as a Physical Therapist, with a focus on home health care.
4. Proficient knowledge of and experience with Rehab and Home Care practices.
5. Experience in conducting Oasis Assessments.
6. Excellent communication and interpersonal skills to interact effectively with patients, families, and healthcare team.
7. Strong organizational and time-management skills, with the ability to manage a caseload of patients.
8. A compassionate and empathetic approach to patient care, with a commitment to improving patients' quality of life.
9. The ability to work independently, make clinical decisions, and problem-solve effectively.
10. A valid driver’s license and reliable transportation to travel to patients' homes within the service area.
11. Current CPR certification.
