Job description

VCU Health at Home by BAYADA is seeking an experienced Physical Therapist (PT) for a full-time home health position providing home visits in South of the River, Richmond, VA.

As a home care Physical Therapist, you will be part of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities include:

  • Making home visits within designated territories.
  • Performing diagnostic tests such as mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
  • Implementing wound prevention and healing interventions using evidence-based dressings and topical agents.
  • Developing and updating individualized care plans, including manual exercises, gait training, and balance interventions.
  • Assessing and revising care plans and collaborating with nursing staff as needed.
  • Educating clients and their families on rehabilitative activities to promote health, safety, and independence.
  • Documenting care activities accurately using electronic tablets.

Qualifications:

  • Current VA Physical Therapist license.
  • Graduation from an accredited program approved by the American Physical Therapy Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Proficiency with computers; EMR experience preferred.
